How to make changes/enroll

Changes can be made only under the following circumstances:
- Within 60-days of hire.
- During annual open enrollment, typically mid Sept - mid Oct.
- Withing 60-days of a "qualifying event". Please note: Proof of your "Qualifying Event" is required.
- What is a "qualifying event?"
- Death
- Birth/Adoption/Foster Care
- Marriage/Divorce/Change in Domestic Partnership
- Loss/Change of Employment/Insurance Coverage
- What is a "qualifying event?"
Documentation needed to Add a family member
Spouse or Domestic Partner:
- Marriage certificate or domestic partnership registration
Natural Child
- Birth certificate naming the employee as parent
Adopted Child
- Adoption certificate or birth/adoption certificate naming the employee (or spouse/domestic partner) as parent
Step-Child
- Birth certificate naming the employee’s spouse as parent
Domestic Partner’s Child
- Birth certificate naming the employee’s domestic partner as parent
Completed CalPERS questionnaire and medical report
Disabled Dependent (Age 26 or Older)
- Authorization to disclose protected health information
- Physician’s statement certifying disability
