How to make changes/enroll

Changes can be made only under the following circumstances:

  1. Within 60-days of hire.
  2. During annual open enrollment, typically mid Sept - mid Oct.
  3. Withing 60-days of a "qualifying event". Please note:  Proof of your "Qualifying Event" is required.
    • What is a "qualifying event?" 
      • Death
      • Birth/Adoption/Foster Care
      • Marriage/Divorce/Change in Domestic Partnership
      • Loss/Change of Employment/Insurance Coverage

Documentation needed to Add a family member

Spouse or Domestic Partner:

  • Marriage certificate or domestic partnership registration

Natural Child

  • Birth certificate naming the employee as parent

Adopted Child

  • Adoption certificate or birth/adoption certificate naming the employee (or spouse/domestic partner) as parent

Step-Child

  • Birth certificate naming the employee’s spouse as parent

Domestic Partner’s Child

  • Birth certificate naming the employee’s domestic partner as parent
    Completed CalPERS questionnaire and medical report

Disabled Dependent (Age 26 or Older) 

  • Authorization to disclose protected health information
  • Physician’s statement certifying disability
